Alert for use Red Rocket Parachute Flare L-35 (Pirolec)

The Dutch Safety Board issues an alert for the use of the Red Rocket Parachute Flare L-35 / L-35A (Pirolec), batch 0525/2021 – 113. This alert arises from the preliminary investigation into an accident on a sailing ship in the Pacific Ocean on 1 January 2023. The flare was fired according to instructions, but exploded immediately with fatal consequences.
The Dutch Safety Board started a preliminary investigation after the accident. It is unclear why the flare exploded immediately. There is a risk that it is not a one-off incident. Therefore the Dutch Safety Board issues an alert as a precaution and calls on all owners of this particular flare from this batch to not use these and to replace them as soon as possible.
