The Dutch Safety Board investigated the accident on the Prins Bernhard bridge in Zaandam, where an elderly couple suffered severe injuries in the evening of 28 November 2018. The bridge was opened while the couple was walking on it. The Safety Board investigated the cause of the accident and how remotely controlled bridges are operated. The report also goes into the Board's recommendations in a report about a similar accident on a remotely controlled bridge (Den Uyl bridge (2015)).
