Safety culture in shipping

ln a relatively short timespan a number of fatal accidents occurred on board of ships. The accidents were related to entering confined spaces, going on deck in bad weather, mooring and unmooring and work in an unsafe workplace. The Dutch Safety Board therefore decided to conduct an investigation into the safety culture in shipping.
 
The investigation focussed on the main question:
"How is it possible that experienced and responsible seafarers do not always take the necessary safety precautions when faced with high risks and what can be done about this?"
 
A separate investigation was carried out into the different accidents. On top of that the Safety Board verified how safety is being considered within the shipping business, both within office organisations of shipowners or managers, and among seafarers themselves.

The results of this investigation will not be published as a report. The information gathered during the investigation will serve as a background study for future investigations of the Dutch Safety Board in the shipping sector.
