Follow-up on the recommendations from the report “Capsizing Frisia”

The Dutch Safety Board has investigated the follow-up on the eight recommendations from the report “Capsizing Frisia”, published in 2012. This report was about the capsizing of a shell-dredger on December 14, 2010, at which all three crewmembers died.
 
The Board decided to initiate a follow-up investigation due to the seriousness of the accident and because serious safety shortcomings had been identified during the investigation. The written reactions on the recommendations did not fully reassure the Board that the chance of reoccurrence had been minimized. The Board has the legal authority to carry out investigation into the follow-up of recommendations. The Board has used that authority in this investigation for the first time.
