Carbon monoxide in bow thruster room, 13 July 2014

On Sunday 13 July 2014 the chief engineer died from carbon monoxide poisoning on board the Netherlands registered motorvessel Lady Irina. The chief engineer passed away in the bow thruster room of the vessel. At the time of the accident the vessel was heading for Kolding (DK) near the harbour of Fredericia. The ship was carrying a cargo of wood pellets. During the rescue attempt, four crew members were also injured by carbon monoxide poisoning. Two of them suffered serious injuries and were transferred to a specialized hospital in Copenhagen.
