Unfortunately, the Dutch Safety Board is unable to look into or assist with problems or complaints affecting individual people. This is because such work would not come under our tasks and competences. All the same, we may be able to make good use of your knowledge and experience. Can I report an incident anonymously?
No, this is not possible, nor is it necessary. This is because investigations into blame or liability are expressly not part of the investigations that the Board carries out. Note that statements made during an investigation, information gathered by the Board, results of technical investigations and analyses, and drafted documents (including the published report) may not be used as evidence in criminal, disciplinary or civil law proceedings and may not be obtained or requested on behalf of third parties.
I have come up with an idea or invention that could improve safety. Would the Board be interested?
It is the Dutch Safety Board’s job to investigate incidents and to make recommendations designed to improve safety. Please submit your idea or invention to those organisations that are better equipped to give it the attention it deserves. These organisations include the appropriate ministry, TNO (the Netherlands Organization for Applied Scientific Research) and certain universities.
I have suffered loss or injury as the result of an accident. Can the Board mediate here?
Unfortunately we are unable to help you. Our competences are defined by law and do not include questions about blame and liability. We recommend that you turn to a claims assessor or to the Victim Support Netherlands organisation.
Can I subscribe to the Dutch Safety Board’s reports?
Yes you can. You can indicate that you wish to stay informed about current investigations by filling in your e-mail adress at the page of the specific investigation that you are interested in.
I have a complaint about a government institution or company. Can the Board assist me with this?
Unfortunately we are unable to help with such matters. If you have a complaint about a government organisation then you should visit the website of the National Ombudsman. This website sets out the criteria used to determine whether the Ombudsman can intervene in a particular dispute with a government organisation. If you have a complaint about a company then you should contact the company itself. Most companies employ a special officer for such work. You may also submit your complaint to the appropriate inspectorate.
I am trying to obtain a copy of an old pre-1999 report, i.e. one that was published before the Transport Safety Board was set up.
You should contact the National Archives or the Ministry for Transport, Public Works and Water Management.
If a short investigation is carried out for which an abridged report is published, will this be followed by a ‘standard’ report at a later date?
No, because after an exploratory investigation has been performed, it will be decided whether a short or a full report is to be published. This depends in part on the seriousness of the incident and on the probable systematic safety shortcomings.
Defence
Which defence incidents are or are not investigated?
Defence is one of the sectors investigated by the Dutch Safety Board. This also covers incidents involving defence planes, shipping, road transport and the underground infrastructure. It does not cover incidents involving acts of war.
Industry, tubes and networks
I want to report an incident that comes under the sector pipelines, industry and networks. What are the appropriate criteria?
Action must be taken depending on the nature of the incident. Please see the webpage Industry, tubes and networks for all the information you need. You can also report the incident at any time day or night by phoning 0800 MELDOVV or 0800-6353688.
I live near to a high-voltage cable / 3G (UMTS) or other transmission mast and am suffering nuisance from it. Can the Board investigate?
Unfortunately we are unable to assist you with such matters. If you have a complaint about a government organisation then you should visit the website of the National Ombudsman. Aviation
I have a question or complaint about noise pollution emanating from flights to or from Schiphol Airport or about air traffic safety. Can you help me?
Complaints about air traffic noise pollution should be submitted to the airport in question. Airports generally have a specific office or department to which you can submit complaints. For questions or complaints about air traffic safety, please contact the Transport, Public Works and Water Management Inspectorate, Flight Operations Inspectorate, Occurrence Analysis Bureau
Where can I get hold of a copy of investigation reports on aviation accidents that occur abroad and that involve Dutch planes?
In principle, such incidents will be investigated by the country in which the incident occurred. It will be the investigating institution of the country in question that ultimately publishes the report.
I have a complaint about an airline. Could you mediate for me?
Unfortunately we are unable to assist you with this. In most cases, airlines have their own complaints office for handling complaints. You can find the relevant details on the airline’s website.
I am looking for statistics on airline incidents. Does your website provide such statistics?
No, but you can find this information on the website of the Transport, Public Works and Water Management Inspectorate at Analyse Bureau Luchtvaartvoorvallen.
Railways
I often see dangerous situations involving overcrowded trains and defective train doors. Is the Board able to investigate such matters?
Unfortunately we are unable to help you here. The best organisation to contact in this regard is the Rail division of the Transport, Public Works and Water Management Inspectorate.
